Hospital-Identified Priorities in Community Health Needs Assessments

Working with the Robert Wood Johnson Foundation, this infographic summarizes the findings of a review of 300 community health needs assessments related to identifying priority health needs and hospital partners in the October 2014 report “Hospital-based Strategies for Creating a Culture of Health.'
