Describe one or two examples of a time when you stepped up to address an issue or challenge in a way that demonstrated your capacity as a leader.

Capstone Project Narrative

The Next Generation Leaders Fellowship requires participants to identify a complex challenge within their organization and/or community for which they will then design a solution guided by peers and mentors. The second half of the program will be spent executing the solution with expert guidance from mentors.

The program will conclude at the AHA Leadership Summit in July 2025 with participants reporting on the impact of their capstone project. If you are selected to participate in the program, you will work with mentors and peers to refine and scale your approach.

For those planning to apply for the John. A. Hartford Foundation Age-Friendly Health Systems Scholarship, your narrative should demonstrate any understanding of the Age-Friendly Health Systems movement and include how to implement and/or spread at least one of the components of the 4M's.
