Improving Health for Older Adults: Chicago South Side Success Story

This webinar highlights how the SHARE Network — a partnership between UChicago Medicine, other health care organizations and community-based organizations —successfully supports healthy aging initiatives and a resource network for adults 65 and older living in Chicago’s South Side neighborhoods. Recognized as an Age-Friendly Health System, UChicago Medicine has improved health outcomes for older adult patients in areas related to medications, mobility, mentation and “what matters” (to patients and their families).

More than 4,000 care sites, including hospitals and health systems, ambulatory care, long-term care and post-acute care, have been recognized as Age-Friendly Health Systems.
