Los Alamitos Medical Center, a 167-bed facility in Los Alamitos, California, used best practices and peer-to-peer learning to reduce harm across the board. By benchmarking data, improving patient safety reporting systems, and improving processes for communicating--with shift-to-shift rounds, daily huddles and 'tempo' boards--the hospital prevented 108 harms in one year.
