This webinar introduces the Age-Friendly Health Systems (AFHS) movement and the 4Ms Framework, What Matters, Medication, Mentation, and Mobility, and explores how aligning AFHS with broader efforts can strengthen care delivery. Learn how Novant Health used data, team-based strategies, and quality improvement principles to drive adoption, overcome challenges, and build momentum for culture change in the care of older adults.
