Case Study: Leading a Communitywide Approach for Age-Friendly Care

The age-friendly task force at AdventHealth Hendersonville created a vision board that helps drive improvements in caring for older adult patients. The health system works with teams across its network as well as with community partners, includes age-friendly initiatives as part of its annual strategic and financial planning, and incorporates age-friendly care into annual wellness visits for older adult patients.
