The American Hospital Association (AHA) Section for Long-Term Care and Rehabilitation invites you to join your colleagues, Douglas Struyk, president and CEO, and Marianne Guerriero, nurse executive, and members of their staff team from Christian Health Care Center, Wyckoff, N.J., on a webinar highlighting their unique approach to providing Advanced Care Planning in the Post-Acute Care environment. Speakers will share thoughts on:

  • How to develop a program for clinical care team members to attain facility-specific certification in Advanced Care Planning;
  • Knowledge and skill set necessary to have conversations with patients and families related to end of life wishes including the use of the Practitioner Orders for Life-Sustaining Treatment (POLST);
  • How to successfully implement the program, including educational sessions for patients and families.
