Members-Only Conference Call: Case Management of Psychiatric Disorders Reduces Readmissions of Medical/Surgical Patients

Shastri Swaminathan, M.D., Chairman, Department of Psychiatry, and Sheri Richardt, LCSW, Manager, Crisis/First Access/MICCS, Advocate Illinois Masonic Medical Center, Chicago, IL, will describe the impact of their Medically Integrated Crisis Community Support (MICCS) initiative, which moves care out of the emergency department (ED) and into the community.
Webinar participants will learn how MICCS was designed and implemented. The initiative initially focused on psychiatric patients who were frequent users of ED services. It has reduced 30 day psychiatric readmissions as well as significantly reduced ED visits of the high utilizers.
Participants will then hear about the next phase and outcomes of this initiative, as MICCS was extended to patients discharged with a medical/surgical diagnosis. Dr. Swaminathan and Ms. Richardt will share how MICCS has improved access to and quality of services, while reducing costs and inappropriate use of the ED.
