The AHA has announced the hospital-community partnership teams participating in the 2021 Hospital Community Collaborative. The collaborative is made up of hospital and community groups from across the U.S. and represents America’s expanding racial diversity, with 20 hospital-community teams. Teams are tasked with developing and executing innovative solutions for creating the conditions people need to live happy, healthy lives. They will be addressing a wide range of social factors, including homelessness, food insecurity, access and safety. Learn more about the teams and their projects here.
