Deadline for states to apply for social determinants of health grants approaching
July 6 is the deadline for state and local governments to apply for grants of up to $125,000 each to develop multi-sector plans to address social determinants of health to improve chronic disease outcomes in populations experiencing health disparities and inequities. The agency expects to fund about 20 jurisdictions through AHA-supported legislation included in the Consolidated Appropriations Act of 2020.
