Porter Medical Center - Primary Care Expansion/Community Health Team

The Porter Hospital board approved a plan to relocate and expand the hospital’s primary care practice in Bristol into the new “Bristol Works!” redevelopment complex. In August 2012, Porter completed the relocation and began welcoming new physicians. The new office has more exam rooms and space for the new “Community Health Team,” which provides services consistent with the Vermont Blueprint for Health initiative. This initiative aims to provide better coordination of services for patients as well as offer education, support and resources designed to help patients with chronic health conditions better manage their disease and improve their health status.

Why do they do it?

An Area Health Education Center study showed a shortage of internal medicine and family physicians throughout the state.

Impact

Through this project, Porter Hospital was able to re-open its practice to new patients and help facilitate access to primary care for area residents.
