On behalf of the American Hospital Association (AHA) Section for Long-Term Care and Rehabilitation, you are invited to join a group of your colleagues for a call and discussion with leadership from the University of Maryland Rehabilitation & Orthopaedic Institute and KentuckyOne Rehabilitation and long-term care hospitals. Cynthia Kelleher, President and CEO of the University of Maryland Rehabilitation & Orthopaedic Institute and Executive Sponsor of the Rehab Network of Baltimore and Randy Napier, President of Southern Indiana Rehab Hospital, New Albany, IN and the Frazier Rehab Institute, KentuckyOne Health, Louisville, and Tonja Williams, president & CEO, Continuing Care Hospital, KentuckyOne Health, Lexington, will describe their use of patient navigation, transitional care teams, and internal education strategies to successfully integrate various levels of Post-acute care in their health care systems and the surrounding community.
