AHA-AMGA Collaborative - Clinical Integration: Pathway to Value

The AHA and the AMGA invite members to apply for the next iteration of our clinical integration learning collaborative. As we strive to deliver smarter, better, safer care, it’s imperative that health care organizations are experienced in developing plans for more integrated systems of care.

The inaugural class consisted of more than 20 teams that developed plans for more integrated care specific to their organizations and the call for the class of 2018-2019 is open!  The program begins in February 2018 and concludes in June 2019.

The AHA-AMGA Learning Collaborative, Clinical Integration Pathway to Value, is a national-level comprehensive program that addresses critical success factors such as leadership, culture, infrastructure, execution, community partnership, physician compensation, communication and performance improvement. Participants learn the skills and information needed to develop a more affordable and equitable system that advances health in America. It is designed to help hospitals and medical groups work together to address the health care needs of their communities in innovative ways within the current payment and incentive system and plan for the future.

Participants will guide their organizations from more traditional payment models to organized, integrated systems of care delivery that manage population health through their team projects. For example, participants will build a business case for participation in a value-based payment contract or a plan for implementation of such a program. The collaborative includes webinars, moderated virtual discussion groups, in-person educational sessions with national experts, successful practice case studies, and a robust peer-learning network.
