The Centers for Medicare & Medicaid Services has issued the 2025-2026 Medicaid Managed Care Rate Development Guide for states to use when setting managed care capitation rates for the rating periods between July 1, 2025, and June 30, 2026. The guide details federal Medicaid rate development standards and information required from states and actuaries, including state-directed payment information that must be included in managed care rate certifications for CMS review and approval.
