The Coalition to Transform Advanced Care (C-TAC), whose members include the AHA, today launched a national campaign to transform advanced illness care through policy change, state and community organizing, and support for family caregivers. According to the coalition, the Campaign to Transform Advanced Care will pursue a policy agenda focused on “delivery system reform, preference-driven care, caregiver and consumer support, and professional engagement,” with the primary goal of establishing a Medicare advanced care demonstration. At the launch event in Washington, D.C., AHA Chief Medical Officer Jay Bhatt, D.O., said AHA will continue to urge Congress and the Department of Health and Human Services to support efforts that incorporate advanced illness management into new models of care. AHA President Emeritus Rich Umbdenstock is a member of the C-TAC board of directors.
