Description

The AHA Center for Health Innovation is looking for AHA member hospitals and health systems to join its new Hospital Community Collaborative (HCC) cohort. Participating health care providers and their partners will receive unique access to vital tools and resources to address social factors that have been putting the health of their communities at risk, especially during the pandemic. Applications are being accepted now through April 30. Learn more at the HCC info session, where the AHA population health team will share a preview of the program’s learning objectives, logistics and application process.
