Collaboration = Better Health

United Memorial Medical Center, Batavia, N.Y., focused on patients with congestive heart failure, chronic obstructive pulmonary disease and pnuemonia and their health needs in order to reduce frequent hospitalizations and readmissions. In order to successfully implement a solution, the hospital worked with the P2 Collaborative of Western New York and the Genesee County Office for Aging to identify patients and provide them with appropriate care.
