Avista Adventist Hospital, a 114-bed facility in Louisville, Colorado, used best practices and peer-to-peer learning to reduce harm across the board. By focusing on engaging all staff and the board, improving care processes and using storytelling, the hospital had a 53.7 percent reduction in patient harm and a 100 percent reduction in central line-associated bloodstream infections. Significant results also were achieved by minimizing adverse drug events and reducing falls with injury. Avista Adventist Hospital was part of the AHA/HRET Hospital Engagement Network.
