Lean Six Sigma tools were used to reduce pressure ulcers at Memorial Health System, Springfield, Ill. Bundles and communication tools were employed.

The hospital set a goal to reduced pressure ulcers by 50 percent by applying Lean Six Sigma methodologies. Process defects were identified for pressure ulcer prevention, including identifying at-risk patients, initiating interventions, achieving minimal turning requirements and effective care coordination across units and shifts. Key improvements include:

  • Development of a 'PUP Kit' consisting of a bundle and bedside communication tools;
  • Creation of a 'team-up to turn' approach to achieve minimal turning requirements with support tools; and
  • Enhanced role definition and training for unit-based wound ostomy champions.

Hospital-acquired pressure ulcer prevalence rates for Stages I-IV ulcers have decreased 79 percent, exceeding the initial goal. Read the whole case study below (click 'view item').
