Cheshire Medical Center/Dartmouth-Hitchcock Keene - Healthiest Community Initiative - Healthy Monadnock 2020

In 2006, the hospital-led Healthiest Community Initiative – Healthy Monadnock 2020 (HM2020) initiative was formed to engage the community in identifying needs, setting ambitious goals and implementing evidence-based, environmental strategies to improve total population health outcomes. For example, in October 2012 the Healthy Eating and Active Living collaborative began assessing wellness activities at work sites and assisting employers to implement programs, projects and policies that foster healthy lifestyle behaviors.

What is it?

In 2006, the hospital-led Healthiest Community Initiative – Healthy Monadnock 2020 (HM2020) initiative was formed to engage the community in identifying needs, setting ambitious goals and implementing evidence-based, environmental strategies to improve total population health outcomes. Who is it for?

The 102,000 people of the Monadnock region.

Why do they do it?

A critical part of the hospitals’ total population health strategy is to address the root causes of disease by changing the environments in which we live, learn, work and play, making the healthiest choice the easiest choice for everyone.

Impact

HM2020 indicators being tracked such as fruit and vegetable consumption, exercise, socializing with friends and volunteerism rates have all increased from 2010 to 2012.
