Virginia Mason Medical Center - Intensive Outpatient Care Program

Virginia Mason launched its Intensive Outpatient Care Program (IOCP) in 2007 to substantially reduce total spending for Boeing's employees and their adult dependents with the highest health care costs. By coordinating care for these patients - including proactive outreach, education in self-management of chronic conditions, and direct involvement of specialists in primary care - the IOCP achieved better health outcomes and increased satisfaction at a lower cost.

What is it?

Virginia Mason launched its Intensive Outpatient Care Program (IOCP) in 2007 to substantially reduce total spending for Boeing's employees and their adult dependents with the highest health care costs. By coordinating care for these patients - including proactive outreach, education in self-management of chronic conditions, and direct involvement of specialists in primary care - the IOCP achieved better health outcomes and increased satisfaction at a lower cost.

Who is it for?

Patients with chronic and complex conditions.

Why do they do it?

Care for these patients is typically uncoordinated, which often results in costly, frequent, and less effective services.

Impact

The pilot achieved positive results in patients' health, functional status, and satisfaction with care. It also helped reduce costs by more than 30 percent, primarily by decreasing hospital admissions and emergency visits.
