Adena Health System - Community Health Partners

The Adena Health Foundation of the Adena Health System began in 2004 to award competitive grants to other non-profit community organizations. Funding for the Community Health Partners Program comes from donations and the health system budget. Grantees are required to include educational components and evaluation plans to measure outcomes at six, 12, and 18 months. A symposium allows all grantees to present their projects.

Why do they do it?

The purpose of the Community Health Partners Program is to provide financial assistance and sometimes hospital staff expertise to organizations that conduct new activities to improve health among their client populations.

Impact

Grant examples include the following projects:

  • A “Health Literacy Project” by the Community Action Committee of Pike County provides education and printed materials to Head Start parents to help them better manage the health care needs of their children and reduce unnecessary emergency department visits.
  • An “Alzheimer’s Caregiver Training” project by Traditions of Chillicothe provides training to family caregivers and staff of area National Church Residences facilities.
  • A “Sisters in Health” project by Guardian Angel Community Health coordinates an eight-week walking and diet awareness program for women primarily of ethnic minorities, and conducts a Sister-to-Sister National Women’s Heart Day Health Fair.
