Rawlins County Health Center - Cardiac Rehabilitation Phase III Wellness Program

This individualized program is for patients who have recently graduated from the Phase II program or have completed a similar monitored training program elsewhere. In addition, individuals with elevated blood pressure, cholesterol, peripheral vascular disease and/or risk factors are excellent candidates for this program. Participants in Phase III are more independent, require less supervision as each individual monitors their own exercise intensity, and become confident with their own exercise patterns safely.

What is it?

Who is it for?

The aging population of Rawlins.

Why do they do it?

This program was created when Rawlins County Health Center realized a need for participants to continue an independent program after a cardiac event. As the need grew, the program was opened up to more participants who just wanted to improve their lives through a structured fitness program. Some of the participants have been coming for years, others only come during the winter months when it is too cold outdoors.

Impact

Rawlins County Health Center currently has up to 43 participants from age 50 to 90 years old.
